Virginia Code §54.1-3427 requires that patients requesting non-safety closure containers sign a release that is then retained by their pharmacy. The law does state the release must cover a period of time; it does not state, however, if that period of time can be indefinite or has an expiration.
The download below can be used as a form to satisfy this requirement; once completed it should be placed in the patient file and retained.
Alternatively, you may complete an online version of the form; note that to complete the form you will need access to a printer as the completed waiver will be generated as a PDF and emailed to the address entered in the form.
Submissions are not retained
Note that form submissions are not retained in the database (both for storage and HIPAA reasons); once completed the form cannot be viewed or retrieved again. If you do not receive the email with the PDF, check your spam folder or complete the form again and add [email protected] to your whitelist.
